Honor Roll Enrollment Form

Parishioners, friends, and neighbors are invited to remember and honor their loved ones through our Honor Roll program.

Anyone interested in enrolling an honoree can complete this form. When submitting the form, you will receive a notification to make a one-time or recurring gift using your debit or credit card. The notification will also include options for making a safe and secure gift.

Or, you can choose to print the PDF form to be filled out and mailed, with accompanying check or cash, to the Parish Office, PO Box 8, Saint Meinrad, IN 47577-0008, or dropped by the Parish Office during regular office hours at 19630 N. 4th Street, Saint Meinrad, IN. The entrance to the Parish Office is located on Brewery Street, on the north side of the building. The convenient, accessible area is located in the carport, equipped with an after-hours drop box.

Throughout the year, parishioners may use the designated envelopes included in the monthly envelope packet, but are encouraged to complete this online form before sending your gift. If your gift is received without the form (names of your honor roll honorees), a link will be forwarded to you by email. This ensures better accuracy of your information.

Your loved ones will be recognized throughout the year, as honor rolls are scheduled, on placards located in the parish church and on the website and in the parish bulletin.
